Donor-Advised Fund

Choose a name for your fund. This will be used when we make donations or investments from the fund, (unless you choose to make them anonymously). Typically the fund is named after your family, your business, or to reflect a particular cause. However it should not conflict with or cause confusion with existing organizations or funds.

Advisor

The advisor (often, but not necessarily, the primary donor into the fund) may provide advice on how the funds are deployed for charitable purpose.

We are required to be able to confirm your identity and address for ‘Know Your Client’ reasons, and may ask for proof – typically in the form of copies of your passport and driving license (Do not send these now, unless requested).

Joint Advisor

You may optionally specify a joint advisor. We may take advice from them, and they have equal standing as the primary Advisor. Should the primary Advisor die then they will become the primary Advisor.

Residual advice

In the event that there are no remaining advisors, or we are unable to reach any advisors, you may specify the principles by which you would wish the fund to be used. Typically this is by reference to particular types of cause, or parts of the world.

Acknowledgement

I understand that any donations to the fund are irrevocable and unconditional contributions when received and accepted by SharedImpact, and that SharedImpact retains exclusive legal control over contributed assets. I certify that to the best of my knowledge all information in connection with this application is true and complete and I will notify SharedImpact in writing of any changes. I certify that I will not provide advice that would lead to any private benefit to donors, advisors, or any connected persons.
